Physiotherapists are primary contact clinicians, and as such, no referral is needed.

Initial consultations are booked for 45 minutes, to allow us to collect a full history, obtain copies of documentation, and complete a thorough physical examination.

In some cases, usually, where there is an extensive history, complex presentations or multiple sites of injury, your physiotherapist may (in consultation with you) complete your initial consultation over two visits to ensure that we have a full understanding of your presentation.

Review consultations are booked for 30 minutes and are targeted to treat a single area. If multiple areas require treatment, a long review consultation can be booked for 45 minutes.

 

Generally, you should wear something in which you are comfortable and allows you to move freely. Your physiotherapist will often need to see the affected area to treat it effectively and clothing that allows the area to be exposed is helpful.

If you are uncomfortable with exposing any area for any reason, please do not hesitate to let us know.

Yes, if you have a referral or discharge summary, any scans (X-rays, MRIs, ultrasounds, etc.), TAC or WorkCover / WorkSafe documentation or any important information related to your presenting condition, please bring them along.

We will always aim to communicate with your GP to ensure that they have a full and accurate record of your medical history. If, for any reason, you wish for us not to communicate with them, please let us know.

Private health insurance claims can be processed onsite at Form + Function Physiotherapy, provided physiotherapy is covered by your policy. If you are unsure it is best to contact your health insurer to confirm the details of your policy.

Payment can be made by credit card on the day of your consultation. We use a payment processing system called Medipass that can process private health insurance claims and credit card payments in one simple process. Details are sent to the patient’s phone for review and payment approval, keeping you in control.

We do not accept cash payments to ensure the safety of our staff.

The payment of accounts is appreciated on the day of consultation.

Diagnostic imaging is not required before attending a physiotherapy appointment. If such imaging is required a referral can be made by your physiotherapist.

Often imaging is not needed as an accurate diagnosis can be made without it.

Yes, we are very happy to see patients regardless of how their injuries occurred. To see patients under TAC or WorkCover we do require details of your claim number, so please bring this to your first appointment.

Our standard fees apply for initial and follow-up consultations and a gap payment may be payable.

There is ample street parking available on Waverley, Swan and Docker Streets, which surround our location. Please observe all parking restrictions as they are strictly enforced in the area.
