It is with great sadness that we are closing Form + Function Physiotherapy as of December 20th, 2020.

This year has been challenging for many businesses and Form + Function Physiotherapy has been no exception. Given the prolonged period of closure and the inability, or marked restrictions on our ability to provide care to our patients we have made the tough decision to close.
